Halo 3 mythic disk won't load maps?

when i pop in disk some times it says unreadable so i look at the disk its clean but i put it back in and i turn on but still cant load maps then i wne to cg loby and look at maps and it only had sand strap and long shore should it be replased or just a messed up seting
